Juniper Evidence-Based Health Promotion Programs for Aging Well

Event Description    
Research shows that 30% of what drives an individual’s health are their health behaviors. Juniper classes assist clients in identifying and changing these health behaviors to improve their overall health and wellbeing. This PowerPoint presentation will provide information about Juniper’s health promotion programs and how they can benefit clients treated by occupational therapists. We will share information and data about specific evidence-based programs offered by Juniper that help people age well. These programs aim to help clients living with chronic conditions or who have concerns about falling. This information will be brought to life by true stories of actual program participants. We will end by sharing how occupational therapists can refer to Juniper programs and how this will benefit their practices.


Speaker Biographies
Ginger Carroll, MS, OT/L
My experience working as an occupational therapist includes the continuum of care with Courage Kenny Rehabilitation Institute starting with inpatient rehabilitation. I worked at Abbott Northwestern Hospital for 12 years both in inpatient as well as acute care. Additionally, I spent some time working home care and believe strongly that meeting people where they are is a skill and privilege that we have as occupational therapists. Most recently I worked the past 5 years with the Courage Kenny Research Center as a project manager. It was an amazing experience to work alongside accomplished practitioners such as Mary Radomski, PhD, OTR/L while conducting research to support our profession. This experience has translated well to helping all people access evidence-based wellness interventions through working with Trellis/Juniper and the National Diabetes Prevention Program.

Maggie Sills, OTD Student
Prior to being admitted to the occupational therapy program at the University of Minnesota, I interned with an occupational therapist for 200 hours at a skilled nursing facility. It was there that I learned the incredible value of occupational therapy in enabling clients to maximize their ability to participate in meaningful activities to promote overall health and wellbeing. While in graduate school, my first 12-week, level 2 fieldwork experience consisted of providing occupational therapy services at an emerging practice site, Open Circle, which is an adult day program. Open Circle promotes healthy living and personal independence. Here, I implemented health promotion techniques by helping to increase or maintain clients' functional capacity, maintain or improve their self-care routines, while assisting to stimulate their social environment through creative, person-centered interventions.

Lauren Peterson, MA
Lauren has been with Trellis on the Juniper program in the role of Strategic Partnerships Manager since November of 2021. Lauren manages relationships between evidence-based health promotion class providers and Trellis and manages the work of Community Health Workers (CHWs). Prior to coming to Trellis, Lauren was at UCare where she worked in a few different roles during her eight-year tenure, including as a Provider Contract Manager, Senior with responsibility for negotiating agreements and maintaining relationships with some of the major Minnesota care systems. She earned a bachelor’s degree from Bethel University and a master’s degree from Luther Seminary. Lauren and her husband live in St Anthony Village and have four children, ages 10, 12, 21 & 22, and a corgi. In her spare time, Lauren enjoys yoga, meditation, and hiking with her family.

    1    Gain an understanding of how health promotion classes can improve the lives of aging clients.
    2    Be able to describe the health promotion programs offered by Juniper
    3    Explore how referrals into the Juniper health promotion programs can benefit your practice

Diversity, Equity, and Inclusion
We will discuss Juniper’s CHW program and Wellness Engagement Center and how those programs work to remove client’s barriers to health promotion program participation by screening for social determinants of health (SDoH) and resource connection.
